Transitioning Patients from Curative to Palliative Care

As part of our home health care program, we created the Bridge Program. Our Bridge Program provides patients thoughtful continuity of care.

Sometimes when a patient transitions from skilled home health care to hospice care, other agencies introduce a new team of health care providers causing undue stress on the patient and his or her family. Patients enrolled in our home health care who transition to end-of-life care may expect the same team of caring health professionals to provide treatment.

Home Health Care Eligibility

Those needing home care services should be:

  • Unable to leave home without a considerable and taxing effort
  • Under the care of a physician
  • Receiving services under a plan of care established and periodically reviewed by a physician
  • In need of physical therapy, speech-language pathology, continuing occupational therapy or skilled nursing care on an intermittent basis
